RN Clinical Supervisor
The RN Clinical Supervisor serves as the Home Health/Hospice Clinical Team lead and is responsible for supervising the day-to-day clinical operational activities of the team. The RN Clinical Supervisor monitors quality, service, and utilization standards and ensures compliance with federal, state, and local regulations. In collaboration with the DPCS, ensures superior care and service delivery. Holds self and staff accountable for creating a culture of honesty, respect, integrity, and trust.

Staff Management & Development
- Manages the day-to-day clinical operations of the hospice team, including practice standards, staffing, and quality improvement.
- Provides overall supervision of Home Health/Hospice nurses and aides.
- Supervises the employee safety programs and risk management. Fosters a culture of honesty, respect, integrity, and trust, and creates a sense of belonging.
- Develops clinical services processes, documentation, and user manuals.
- Plans and implements in-service and continuing education training programs and keeping staff up to date with current clinical information and practices. Acts as resource to staff as clinical expert.
- Develops and implements action plans to improve staff development.
- Participates in interviewing, hiring, monitoring assignments, and evaluating personnel in collaboration with the DPCS.
- Responsible for new Hospice staff orientation.
- Evaluates the performance of program staff as needed, including annual evaluations. Evaluates the competency of clinical staff with periodic supervisory visits.
- Coordinates and participates in on-call responsibilities for the department. Coordinates all patient care and services, in collaboration with the Director of Patient Care Services.
Quality Assurance
- Collects and organizes all patient-specific information needed to determine the suitability of patients for hospice. Assesses patients' suitability for hospice in accordance with department policies. Verifies that the patient's medical condition and prescribed therapy is suitable for hospice.
- Ensures patients receive quality care by reviewing documentation, making visits with staff, reviewing care plans, conducting/participating in multidisciplinary team conferences, and reviewing customer satisfaction reports. Investigates and resolves patient/family member concerns regarding patient care.
- Ensures the highest quality of care is provided and is in compliance with federal, state, and local regulatory requirements and established departmental policies and procedures.
- Applies continuous quality improvement and total quality management principles.
- Monitors the quality of service and utilization standards and assumes specific responsibilities for patient care at the agency level.
- Ensures individualized care goals are met.
Planning & Strategy
- Participates in Home Health/Hospice program management, including licensing, budgeting, utilization, and quality assessment/improvement activities.
- Assists in the preparation and development of annual budget and organizational goals related to hospice care and services.
- Ensures coordinated plans of treatment, client-driven care, and cost-effective utilization of services.
- Monitors the allocation and utilization of personnel based on continual changes in patient population/needs and provides the best level of patient care while identifying cost-saving opportunities.
- Consults with the DPCS to determine a staffing pattern which will meet the agency's objectives and promote maximum utilization of field staff.
Marketing & Community Development
- Works with healthcare providers outside of the agency to achieve optimal patient care across the care continuum.
- Coordinates with appropriate team members or contract services for the clinical care of hospice patients and families.
- Assists in marketing the agency and fostering community relations.
Regulatory Compliance
- Adheres to all Home Health and Hospice-related standards and regulations at local, state, and federal levels.
- Develops, updates, and maintains Hospice policy and procedures manuals. Ensures that the agency operates in compliance with legal and regulatory requirements.
- Works with DPCS to ensure compliance with contracted agencies and prepare for evaluations and audits to maintain credential status.
- Assures 24-hour clinical coverage for hospice patients in collaboration with the DPCS.
Other Job Functions/Responsibilities
- In the absence of the DPCS will become the acting DPCS and will be vested with the authority to act on behalf of the DPCS.
- Other duties as assigned by the DPCS or the Administrator.
- Prefer a minimum of five (5) years of nursing experience, two (2) of which are in the last five (5) years as a Registered Nurse in a Home Health & Hospice agency, primary care clinic, or health facility.
