Licensed Vocational Nurse (LVN)
We are seeking a compassionate and reliable Licensed Vocational Nurse (LVN) to join our team. The LVN will provide direct nursing care and support to patients under the supervision of a physician, ensuring high-quality, patient-centered care that aligns with our organization's standards and regulatory requirements.
Essential Duties and Responsibilities:
- Provide basic nursing care and administer medications and treatments as prescribed.
- Process medication refills in accordance with established protocols and under the direction of the provider.
- Monitor and document patients' vital signs, symptoms, and responses to treatment.
- Assist with patient assessments and report changes in condition to the provider team promptly.
- Maintain accurate and timely medical records in accordance with facility policies and state/federal regulations.
- Support patient education and promote health and wellness within the scope of licensure.
- Coordinate with multidisciplinary team members to ensure continuity of care.
- Maintain a safe and clean working environment; adhere to infection control and safety protocols.
- Participate in staff meetings and quality improvement initiatives as required.
Qualifications:
- Current and valid LVN license in the state of California
- CPR/BLS certification
- Experience in a clinical, long-term care, or outpatient setting preferred
- Strong communication, documentation, and organizational skills.
- Dependable and dedicated to providing quality patient care.
Language skills:
- Must possess excellent organization, writing, and verbal skills
- English proficiency required
Reasoning Ability:
- Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
Equipment/Machinery:
Fax machine, copier, computer, telephone, EHR system, sterilization equipment and other software and instruments as required.
