Case Manager
This position involves supporting complex cases across the system and patients in the community virtually. Floating between different offices may be required, but the primary work will be virtual. The candidate must have inpatient case management experience. The job requires knowledge of acute hospital care, care coordination, and discharge planning. Certification in Basic Life Support (BLS) is necessary. Familiarity with reviewing medical necessity and understanding CMS guidelines are essential.
Experience with Inter Qual criteria and concurrent review processes is advantageous. The role emphasizes effective communication and coordination with interdisciplinary support services.
