You will collaborate with physicians and the IDT to manage ongoing care, administer medications, and coordinate tests and treatments in a busy primary care setting.
You will perform comprehensive nursing assessments, contribute to care planning, educate Participants and families, and support care transitions, telehealth, and end-of-life
