Position: CLINICAL SOCIAL WORKER

Compensation: $56,000 annually (0.8 Full-Time Equivalent). 30 hours per week, flexible. Benefits include medical, dental, vision, long-term disability, life insurance, optional retirement, flexible spending account, and commuter benefits.

Founded in 1983, Dolores Street Community Services (DSCS) is a multi-issue, multi-strategy nonprofit organization, with an annual budget of $14 million, based in San Francisco's Mission District. About the Program:

The Richard M. Cohen Residence is a Transitional Residential Care Facility (TRCF) located in the Mission Dolores neighborhood of San Francisco. The program provides housing and supportive services to 10 individuals living with HIV/AIDS, many of whom were formerly homeless. Services include health education, case management, housing navigation, community activities, and food/meal support. The program promotes self-sufficiency and client empowerment, working with each person to identify individualized goals for achieving long-term housing.

Position Summary:

The Clinical Social Worker is responsible for care coordination, housing navigation, and therapeutic support to residents of the Richard M. Cohen Residence. The Social Worker supports residents by coordinating medical appointments and referrals, conducting individual behavioral health sessions, assisting with obtaining long-term housing in collaboration with the Lead Health Promoter, and providing clinical guidance and training to staff. The position reports to the Program Director and collaborates with a team of Health Promoters.

Responsibilities:

  • Conduct comprehensive psychosocial assessments for all residents and annual reassessments.
  • Develop and update individualized service plans to improve health and secure long-term housing.
  • Document all data in client charts and the DPH reporting system.
  • Communicate regularly with residents' primary care teams to ensure adherence to appointments and medications.
  • Provide appointment reminders, health education, and treatment adherence support.
  • Assess residents' psychosocial needs and provide education, counseling, advocacy, or referrals as appropriate.
  • Coordinate additional services such as IHSS, financial management, or meal preparation.
  • Assist with housing navigation, including identifying opportunities, completing applications, and supporting the moving process.
  • Lead educational and training sessions on clinical and psychosocial issues.
  • Coordinate and plan community activities with the program team.
  • Participate in staff training, meetings, and development activities.
  • Perform other duties as assigned.
  • Position Requirements:

    • 3+ years of experience in social work, counseling, or a related field.
    • Licensed clinical degree (e.g., LCSW) preferred.
    • Experience working with individuals living with HIV/AIDS, mental illness, homelessness, or substance use.
    • Ability to set boundaries and manage challenging behaviors.
    • Bilingual in Spanish preferred but not required.
    • Experience with crisis intervention, conflict resolution, and de-escalation.
    • Experience assisting clients with navigating public benefits or housing is preferred.
    • Strong communication skills, both written and oral.
    • Ability to work independently and in a team.
    • Proficiency with Microsoft Word, Outlook, and Excel.
    • Understanding of intersectionality and the needs of communities impacted by homelessness in San Francisco.
    • Commitment to housing justice and inclusive, multicultural programming.
