To implement and deliver all aspects of peer support and to assist program participants in achieving short‑ and long‑term goals.”

Responsibilities

The Program Manager will be responsible for overall ADSP Program Staff Supervision, advocacy and support services to eligible families and individuals for a caseload of 12‑15 families/individuals. The Program Coordinator also helps people access other applicable housing opportunities and assists clients in maintaining or increasing their housing stabilization.

Working to assess the participants' needs, by providing immediate services, personal goals, and service plans. This position will work with participants around self‑sufficiency, and how to maintain independence and prevent homelessness. Through the program, clients may receive case management referrals to community support and counseling.

The Program Manager provides advocacy and referral services as needed to enrolled clients, and where possible, assist with maintaining housing.

Program Intake, Enrollment, and Application Management
  • Serve as the primary point of contact for new applications and program enrollments submitted through the DATAVERSE system by partner agencies.
  • Receive, review, and process new applications for subsidy enrollments.
  • Review case management applications and supporting documentation to ensure they are complete, accurate, and in compliance with Catholic Charities and MOHCD/Subsidy Program policy and procedure prior to enrollment.
  • Create and manage new client enrollments in the program roster, CARES (internal client management system), and the GMS system (funding system).
  • Divide new applications into case managers in coordination with the Senior Program Director.
Case Management Support & Client Services
  • Provide comprehensive screening and assessment of clients to determine service needs.
  • In collaboration with the Senior Program Director, provide consultation and guidance to case managers regarding application processing, policy clarification, and client inquiries.
  • Provide ongoing client support, including information and referrals, regular client contact, and assistance in achieving service plan goals.
  • Arrange and coordinate services to address clients’ immediate and long‑term needs as documented in individualized care plans.
  • Maintain a minimum of monthly contact with clients to support housing stability and life management skills.
  • Develop and maintain person‑centered care plans, including counseling, monitoring progress, and modifying plans as needed.
  • Coordinate housing stabilization plans for assigned families/clients, including:
  • Share other subsidizing housing opportunities and assist the client with the applications.
  • Obtaining and reviewing required landlord documentation.
  • Making accurate and timely referrals to internal and external service providers.
  • Mediating with landlords, property managers, attorneys, or legal representatives to obtain required documentation and prevent eviction.
  • Requesting payments for additional assistance such as back rent, furniture and basic needs.
  • Update program rosters (ADSP and OALTS) to reflect approved changes, additional payments, and client status updates.
  • Generate monthly reports for Accounts Payable to issue subsidy payments (ADSP and OALTS).
  • Process and track monthly check requests for payments outside of subsidies (e.g., basic needs, back rent, first month’s rent, security deposits) for both programs (ADSP and OALTS).
  • Coordinate‑Schedule and deliver special handling checks on the first business day of the month.
  • Compile required funder monthly reports through the GMS system.
Policy, Procedures & Program Administration
  • Assist the Senior Program Director in creating, drafting, and updating program policies, procedures, and forms as required.
