Job Description:
Within a team of five social assistants, contribute to creating conditions for patients to be actors in their development and to strengthen social links and solidarities in living places. Advise, guide, and support patients and their families, help and accompany them in their procedures, inform the services they belong to for the instruction of a social action measure. Coordinate and participate in different clinical meetings, individual and family syntheses.
Build and animate the work in network.
Required skills and knowledge:
Ability to work and share in a multi-professional team within the pole.
Ability to animate and develop a professional network to work in partnership with social and medico-social stakeholders of the sectors.
Ability to organize to manage requests in different places of intervention.
Ability to elaborate and implement an individual project, master individual interview.
Ability to write information related to the care to ensure follow-up and traceability and ability to transmit.
Required qualities:
Rigor, availability, mobility, motivation, assiduity, adaptation and writing skills.
Relational skills and empathy.
Expected SkillsObligatory holder of the French State Diploma of Assistant of Social Service.
