Qualifications

• Meet one of the following education and experience pathways:
• Option A:
  Master’s degree (MA/MS/MSW) in counseling, social work, sociology, or psychology.
• Option B:
  Bachelor’s degree in social work, sociology, counseling, or psychology, with at least 6 months of experience working in a clinical/mental health setting.
• Option C:
  Associate’s degree (AA) in social work, sociology, counseling, or psychology, with at least 4 years of experience working in a clinical/mental health setting.
• Knowledge and application of legal and ethical standards of practice, including child abuse reporting requirements.

· Possess sufficient clinical skills necessary for assessing presence and impact of youth emotional/behavioral needs, conducting appropriate referrals, and building positive and trusting relationships with youth and families

• Valid California driver’s license and access to reliable transportation, or the ability to meet travel requirements through reasonable accommodation.

Position Summary

Under the supervision of the Director of Community Programs and the Associate Director of Community Services, the Clinical Case Manager provides direct case management and therapeutic services in alignment with professional, legal, and ethical standards, as well as the mission and values of the YMCA of San Francisco. The Clinical Case Manager uses empathetic, client-centered, and solution-focused approaches to support youth involved in the juvenile justice system, or at risk of involvement due to truancy, and their families.

The goal is to prevent first-time offender youth and youth struggling with school attendance from further system involvement through supporting youth emotional well-being, areas of life functioning, and future planning. This role requires active participation in required meetings and trainings, and consistent delivery of high-quality client services.

Work schedule is Monday, Tuesday, Thursday and Friday from 9:00am - 5:30pm and Wednesdays from 10:30am – 7:00pm most weeks.

Job Responsibilities

• Deliver case management and therapeutic services for clients in San Mateo Police Department’s Diversion Program and in the Jefferson Elementary School District, including counseling, crisis intervention, skills coaching, and care coordination in community and home settings.
• Partner with schools, law enforcement, and community organizations to coordinate treatment and support for youth and families.
• Engage in individual and group supervision per BBS requirements and maintain clear, timely communication with team members and leadership.
