• Bachelor's degree in social work, Human Services, Education, Public Health, or a related field OR equivalent professional and community experience
• 2+years of experience working with elementary and/or middle school youth in educational, community-based, or youth development settings.
• Strong relationship-building and engagement skills with youth, families, community partners, and staff.
• Demonstrated leadership, organizational, and program coordination skills.
• Strong verbal and written communication skills.
• Ability to provide and receive constructive feedback.
• Commitment to youth development, prevention services, and organizational values.
• Availability to work Saturdays and occasional evenings as required.
• Proficiency with documentation, data collection, and reporting requirements.
• Ability to maintain confidentiality and exercise sound professional judgment.
• TB Test Clearance within the last two years, as required for programs located on school district sites.

Position Summary

Under the supervision of the Director of Youth & Families Programs, the Youth Program Specialist supports the development, coordination, and implementation of youth and family prevention service programs funded through the San Francisco Department of Public Health. This position is responsible for designing and facilitating student transition programs and leadership development opportunities that promote engagement, personal growth, and successful continuity between school-year and summer programming.

This position works closely with students, families, school staff, and community partners to deliver workshops, support case management and prevention services, and ensure a holistic approach to meeting the needs of the school community while maintaining compliance with program, grant, and funding requirements.

• Coordinate, facilitate, and implement evidence-based youth and family prevention service programs in alignment with grant, curriculum, contractual, and funding requirements.
• Design and implement transition and leadership development programs that support student engagement, growth, and success.
• Facilitate workshops, groups, activities, and prevention education sessions that promote positive youth development, family well-being, and community engagement.
• Support Department of Public Health (DPH) programs through the facilitation of Strengthening Families Program (SFP), Life Skills Training (LST), and other assigned prevention education sessions.

• Conduct assessments and develop Family Service Plans using established intervention methods and techniques.
• Provide ongoing support and referrals to youth and families as appropriate.
• Participate in case management team meetings and carry out assigned follow-up actions and responsibilities.
• Support participant recruitment, enrollment, retention, and engagement efforts to achieve program goals and outcomes.
