This fully onsite, client‑facing role requires direct service delivery with no remote work available.
Summary of Duties and ResponsibilitiesProvides short‑term case management services to seniors and adults with disabilities who may be homeless from the central city area. Must manage 40–60 active cases per month and offer assessment, counseling and appropriate referrals. Works with housed and non‑housed clients from the DAAS Office of Aging (OOA) Centralized Intake and Waitlist System as assigned by the CKSC Case Management Program Manager.
Co‑facilitates support group meetings for homeless and formerly homeless consumers. Works with HSH and partner agencies to transition participants into housing or treatment, providing smooth transitions and follow‑up care when necessary.
- Navigate the OOA Centralized Intake and Waitlist System.
- Accept emergent referrals and enroll and assess clients from the OOA Centralized Intake and Waitlist System.
- Conduct initial intake, comprehensive assessment and home visits to gather information on clients’ needs (social, environmental, physical or mental).
- Develop care plans, service arrangements and monitoring/up‑follow plans, refer appropriate clients to Long Term Case Manager when necessary.
- Maintain up‑to‑date documentation in client files, including referrals and supportive services.
- Submit intakes, assessments, client notes, care plans and discharge information into the OOA system with approval from the CKSC Case Management Program Manager.
- Meet Units of Service (UOS) required by the Department of Aging and Adult Services within the contract period.
- Orient clients to case management, grievance policies and procedures, and case management requirements.
- Advocate with outside agencies on behalf of clients to secure benefits, housing and essential resources.
- Follow‑up on referrals by contacting the client and/or agency.
- Develop and maintain professional relationships with community service providers, employees, volunteers and clients.
- Meet with the Clinical Consultant from Institute on Aging regularly to review cases.
- Collaborate with the CKSC Case Management Program Manager to audit client files and system submissions.
- Attend meetings and trainings as needed.
- Perform other duties as assigned.
- BA/BS in Social Work, Counseling, Psychology, Gerontology or a related human services field.
- Experience in case management and social services to older adults, homeless individuals, substance abusers, and/or mentally ill populations.
- Three (3) years of progressively responsible, full‑time, paid experience as a case manager, counselor or client advocate working with homeless, mentally ill or substance‑abusing populations.
- Secure finger‑image screening and annual TB screening.
- Meet physical requirements test, including bending and lifting up to 40 lbs.
- Use computer and telephone; spend long periods at a desk or computer terminal.
- Abide by Episcopal Community Services rules, regulations and confidentiality requirements.
