Epiphany Center provides trauma-informed, family-centered services that support women and families as they build stability, strengthen recovery, and move toward greater independence.

We're looking for an Outpatient Case Manager to join our Family Residence Outpatient Program
. In this role, you'll provide comprehensive case management and ongoing recovery support, helping clients connect with resources, navigate services, develop after-care plans, and work toward their individual goals.

As an Outpatient Case Manager, you'll work with the Family Residence Outpatient Program team to provide comprehensive case management and recovery support to clients and families.

• Provide ongoing case management and individualized support to clients.
• Meet regularly with clients to assess needs, identify goals, and support progress.
• Connect clients with community resources, services, and appropriate referrals.
• Coordinate with community providers and partner agencies, including social services and public health resources.
• Facilitate or support recovery-focused and life-skills groups.
• Develop and support after-care plans to help clients maintain stability and recovery.
• Maintain timely, accurate, and confidential client records and documentation.
• Collaborate with multidisciplinary team members to coordinate services and address client needs.
• Maintain appropriate professional boundaries and client confidentiality.
• Participate in required staff meetings, trainings, and program activities.
• Follow Epiphany Center policies, safety procedures, confidentiality requirements, and trauma-informed practices.

Pay Range: $30.00-$33.00 per hour

This is a full-time, on-site position

Schedule: Monday-Friday, 8:30 a.m.

-5:00 p.m.
Location: San Francisco, CA - On-Site
Program: Family Residence Outpatient Program

Requirements

What You'll Bring

• High school diploma or equivalent.
• At least 2 years of relevant experience working in behavioral health, substance use recovery, case management, social services, or a related setting.
• Current registration or certification as an alcohol and other drug (AOD) counselor through CAADE, CCAPP, or CADTP.
• Strong communication, organization, documentation, and interpersonal skills.
• Ability to work effectively as part of a multidisciplinary, trauma-informed team.
• Ability to establish and maintain professional boundaries while building respectful relationships with clients from diverse backgrounds.
• Basic computer proficiency and ability to maintain timely and accurate documentation.
• CPR and First Aid certification as required for the position.
• Ability to meet all applicable pre-employment and program requirements.
