GLIDE in San Francisco is seeking a Journey Home Case Manager to lead mobile and on-site outreach, intake, and low-threshold to intensive case management for 20-25 clients. You will develop care plans, coordinate shelter and housing resources, and support clients toward housing stability.
The role requires a valid California Driver's license, HIPAA familiarity, and strong communication. Shifts include Sunday-Monday 10:30pm-7:30am, 16 hours per week, part-time.
