The Katie
A. Services Bilingual Care Coordinator provides therapeutic services, including counseling, case management, permanency planning, and crisis intervention, to youth and young adults ages 6-21 years of age. Care Coordinators are members of a treatment team, and provide services at the program site and in the community, including schools, and family homes. Santa Clara County KAS Services aim to address mental health symptoms that are impacting the youth’s ability to successfully meet their life goals.
In addition to symptom management and reduction, clinicians collaborate with youth and their family treatment teams to identify and work toward overall wellness goals, such as increasing independent skills, increasing adaptive coping skills, expanding their support network, accessing community resources, and pursuing employment and educational opportunities.

A. Services services dually focus on the child and family team meeting process that focuses on building a strong team to collaboratively address the needs of the child and family. Services are provided at the program site and in the community, including schools, and family homes.

RESPONSIBILITIES- Coordinate the youth’s Child and Family Team and develop the individualized treatment plan based on the family’s strengths and needs, including safety planning.
- Maintain regular contact with Child and Family Team members
- Provide culturally responsive, trauma-informed and family-centered services.
- Assist the youth and the Child and Family Team to access needed resources and strength-based mental health services, social services, education services, and other supports and services
- Provide on-call crisis/emergency services as needed for youth and families who access the program’s 24 hour crisis phone line.
- Discuss the provision and quality of activities actually provided with the youth and the CFT and ensure that the activities being provided are responsive to goals and needs.
- Provide transportation for the youth to appointments or to access services as required.
- Maintain all required documentation in a timely manner.
- Attend regularly scheduled staff meetings.
- Comply with all requirements of child abuse reporting, Medi-Cal compliance, confidentiality and HIPAA regulations in accordance with standards set by Seneca Family of Agencies.
- Perform any other duties as assigned by the Supervisor or Director of the program.
- Master’s degree in social work/psychology/counseling
- Registered with California BBS: ASW, AMFT, APCC, or full licensed
Experience working with children who are part of the child welfare system; experience working with children under age five a plus
