Home Health LVN

Responsibilities

Under the direction of a Registered Nurse (RN) and in accordance with physician’s orders, the LVN will:

• Provide skilled nursing care in patients’ homes (wound care, medication administration, injections, vital signs, etc.)
• Perform patient assessments; monitor and document health status, progress, and changes; communicate condition changes to RN, Case Manager, or physician as needed
• Collaborate with interdisciplinary care team (RN, Home Health Aides, therapists, social workers) to coordinate services and update care plans
• Educate patients and families on care plans, medication management, and home care instructions to promote patient comfort, independence, and dignity
• Maintain accurate clinical documentation and patient records, in compliance with state regulations and agency policies

• Current, active California LVN license in good standing
• Valid driver’s license and reliable insured vehicle (for home visits) — if travel required
• Minimum of 1 year experience in home health care, community health, hospice, or similar setting preferred
• Proficiency with EMR/EHR systems (or willingness to learn)
• Strong communication and interpersonal skills; compassion, empathy, and ability to work independently in home-based settings
• Ability to collaborate with multi-disciplinary team, follow care plans, and adapt to varied patient needs

• Hourly Rate: $40–$55 per hour (or equivalent salary) — depending on experience and shift
• Mileage reimbursement for home visits
