Job Summary and QualificationsThe Imaging Coordinator is responsible for facilitating patient continuity of care within the imaging department. The Imaging Coordinator assists with planning, organizing, implementing, and evaluating the patient care needs by advocating, educating, and coordinating the patient through their needs within the department.
Functions as a member of the multidisciplinary team and serves as an advocate and educator for patients. The coordinator will serve as a liaison between the patient and family, all physicians involved in that patient’s care, internal and external healthcare providers, support network members, and the wider healthcare community. This role will require collaboration with local physician liaisons and leadership to conduct internal and external outreach and marketing.
Whatyou will do in this role:
- Assesses, plans, implements and evaluates patients according to physician orders, and establishes plan of care in a timely manner.
- Coordinates scheduling for outpatient testing to be performed to aid in diagnosis
- Reviews physician’s orders for scheduled patients in advance of their appointment, ensuring appropriateness of the scheduled tests, identifies need for referring physician’s orders and calls patients/physicians as needed
- Coordinates 6 month follow up visits for applicable patients
- Assists in scheduling mammogram call back patients and patients requiring mammogram contrast studies
- Obtains and uploads outside films and labs when applicable
- Coordinates with physician offices for any clarification of orders or additional requirements
- Responsible for monthly patient letters for recommended imaging follow-up scans and notification of providers and patients if overdue
- Communicates with all members of the healthcare team who are providing patient care
- Liaison between patient and physician offices
- Schedule follow up studies as needed
- Facilitates timely access to quality medical care from receipt of physician order to diagnostic testing results
- Works with oncology nurse navigators for patient hand-off for positive oncology diagnosis
- Collaborates with physicians and other healthcare providers to facilitate process improvements within the interdisciplinary model
- Educates patient and family members on imaging and diagnostic procedures
- Participates in quality improvement projects when applicable
- 1–2-year LPN/LVN diploma; ADN preferred
- Must have a minimum of three (3) years of progressive hospital/clinical experience.
- Experience in high-risk women’s program
- Experience in care coordination and/or patient education
- Experience in mammography and breast imaging modalities
- Experience in customer service or community engagement
- 1-5 years imaging experience
- LVN/LPN minimum;
Registered Nurse (RN) preferred - Note: LVN and LPN are interchangeable titles (LVN in CA/TX; LPN in other states) Basic Life Support (BLS)
