Responsibilities- The Faith Community Nursing (FCN) Coordinator supports the development, implementation, and evaluation of faith community nursing programs in assigned communities.
- Serving as a liaison between the healthcare system, local faith communities, and the community at large.
- The coordinator guides both paid and volunteer FCNs in providing holistic care that integrates physical, emotional, and spiritual well-being.
- Key responsibilities include recruiting and orienting FCNs, assessing health needs, coordinating and performing wellness screenings and education events, as well as presentations to nurses and clergy.
- The coordinator also maintains program data, ensures compliance of data collection from FCN Volunteer and self reporting on Community Benefit Form.
- Fosters partnerships with congregations and community organizations to align services with Bay Care’s health priorities and Community Health Needs Assessments (CHNAs).
- The faith community nurse coordinator is responsible to coordinate care of the clients enrolled in the FCN Transitional Care Program.
- Responsible for transportation between facilities and to community events.
- Other duties as assigned.
- Education:
Associate's degree in Nursing or Diploma Nursing. - Certification: BLS (Basic Life Support) and RN (Registered Nurse).
- Experience:
Minimum 3 years of direct patient care experience. Working knowledge and computer proficiency (Microsoft suite).
- Education:
BSN (Bachelor of Science in Nursing).
