Prohealth is a locally owned Home Health, Hospice & Palliative Care agency with four locations serving over 12 counties in Northern California. We maintain an average daily census of over 1,800 patients and have been an industry leader for over 15 years. Our patient‑focused care model provides personalized care through nursing, physical therapy, occupational therapy, speech therapy, and registered dietitian services.

We seek an empathetic Home Health Registered Nurse (RN) who will make a positive difference in our community by providing skilled and dignified care in the comfort of patients’ homes and offering emotional support to patients and their families.

• Provide admission, evaluation, and follow‑up skilled nursing visits for home health patients
• Provide quality hands‑on care, manage and teach patient care plans in accordance with physician orders under the Clinical Manager’s supervision
• Coordinate appropriate care involving all healthcare team members, including physical therapists, occupational therapists, home health aides, and medical social workers
• Report patient progress to physician and Clinical Manager as required to maintain continuity of care
• Implement plan of care with patient and family to assist them in achieving optimal outcomes

• Valid RN license to practice in California
• Current BLS/CPR certification
• Valid California driver’s license
• Minimum of one year nursing experience (preferred)
• Experience in Med‑Surg, ICU, ER, acute care, or home health is a plus
• Wound care experience preferred
• Language skills in Mandarin, Vietnamese, or Cantonese preferred
