Join our San Jose, California team! Nurse Practitioner | Salary: $158,000 – $185,000

Requirements
  • Master's Degree or higher in Nursing
  • Current California Nurse Practitioner license required
  • Current DEA registration required
  • Minimum of 2 years of clinical experience
  • Bilingual in one of the following languages preferred:
    Mandarin, Cantonese, Spanish, Tagalog, Farsi, or Vietnamese

Full-time or part time (3 days a week), mission-driven advanced practice role providing comprehensive, patient-centered care within an integrated community health setting focused on whole-person wellness and collaborative care.

Job Overview

We are seeking a compassionate and experienced Nurse Practitioner to provide high-quality medical care to patients across multiple clinic locations in San Jose. Working under the supervision of a physician and as part of an integrated care team, this role focuses on delivering comprehensive primary and preventive healthcare services to diverse patient populations.

The ideal candidate is collaborative, culturally responsive, and passionate about serving the community. This individual will assess, diagnose, and treat patients while promoting wellness, preventive care, and chronic disease management in a fast-paced outpatient setting.

Key Responsibilities
  • Conduct physical assessments
  • Review medical histories
  • Prescribe medications
  • Develop treatment plans
  • Educate patients on disease prevention and medication compliance
  • Coordinate referrals to specialists
  • Maintain accurate clinical documentation in compliance with regulatory standards
  • Collaborate closely with behavioral health and wellness teams
  • Participate in quality improvement initiatives

    Support clinical audits and reporting
  • Attend team meetings
  • Provide coverage for other clinicians as needed
  • Participate in an after-hours on-call rotation
