What You'll Be DoingYou'll be at the center of each patient's healthcare journey by:
- Providing hands-on nursing care in a busy outpatient primary care clinic.
- Triaging patient phone calls and walk-in concerns with sound clinical judgment.
- Reviewing lab results and diagnostic testing while coordinating follow-up care with providers.
- Partnering with physicians, behavioral health professionals, and care coordinators to create individualized care plans.
- Helping patients better understand chronic conditions, medications, preventive care, and healthy lifestyle choices.
- Maintaining accurate documentation within the electronic health record (EHR).
- Supporting quality improvement initiatives and ensuring clinic readiness.
- Delivering compassionate, culturally responsive care to a diverse patient community.

Required Qualifications- Bachelor of Science in Nursing (BSN)
- 2+ years of RN experience (outpatient or primary care preferred)
- Current BLS certification
- Strong assessment, communication, and patient education skills
- Experience working within an electronic health record (EHR)
- Collaborative mindset and passion for patient-centered care
- ACLS and/or PALS certification
- Experience in primary care, family medicine, ambulatory care, community health, or FQHC settings
- Bilingual in Spanish, Mandarin, or Vietnamese
