We're seeking Nurse Practitioners to perform in-home Annual Wellness Visits for Medicare members. These visits are conversational, consultative, and education-focused - no prescribing, no treatment plans.

- Conduct comprehensive in-home wellness assessments
- Educate members on preventive care benefits
- Identify potential gaps in care to help reduce avoidable ER visits and hospitalizations
- Complete documentation using provided EMR tools
- Visits are face-to-face and take place where members live - allowing for more personalized, impactful care.
- Active State NP license and Board Certification
- Valid NPI
- Minimum 3 months of wellness exam experience preferred
- Comfort with EMR/documentation systems
- Ability to travel within a region (visits are clustered in a 10 mile radius)
- Bilingual/Spanish Speaking a plus
- Give on average 2 full days per week.
