About the OpportunityA leading healthcare organization providing comprehensive care services for seniors is seeking a Primary Care Provider (Nurse Practitioner or Physician Assistant) to join a collaborative interdisciplinary team. This role focuses on delivering high-quality primary and geriatric care that enables older adults to remain safely in their homes and communities.
Working closely with physicians and care teams, the Primary Care Provider will conduct assessments, develop care plans, coordinate treatment, and support ongoing participant health and wellness.
- Perform comprehensive in-person assessments, including initial, routine, and unscheduled evaluations
- Provide ongoing primary care services to participants
- Collaborate with physicians and interdisciplinary team members to develop and implement individualized care plans
- Participate in discharge planning from hospital and long-term care settings
- Conduct regular visits and provide oversight for participants residing in nursing facilities as needed
- Coordinate care with specialists, community providers, and care team members
- Review clinical outcomes and utilization trends to support quality improvement initiatives
- Participate in a rotating evening and weekend on-call schedule
- Graduate of an accredited Nurse Practitioner or Physician Assistant program
- Current California NP License with Prescriptive Authority or California PA License
- Minimum 2 years of NP or PA experience
- Experience working with senior populations
- Knowledge of chronic disease management, geriatric care, and clinical best practices
- Geriatrics experience
- Experience in managed care, primary care, or value-based care settings
- Strong interdisciplinary team collaboration skills
- Education
- Master's Degree required
- Experience
- 2 to 5 years of relevant clinical experience
- Mid-Senior level
