Case Manager LVN-LPN

Human Good community is seeking a Case Manager LVN-LPN to provide compassionate, person-centered nursing care for older adults in our senior living community. In this role, you will complete clinical assessments, develop and update care plans, administer medications and treatments, and coordinate services with an interdisciplinary team and outside providers. You will closely monitor residents' health status, maintain accurate clinical documentation, and support safe transitions of care.

Responsibilities

• Conduct nursing assessments and develop individualized care plans for senior residents.
• Administer medications and treatments following physician orders and community protocols.
• Coordinate care and services with interdisciplinary team members and external providers.
• Monitor resident health status, document changes, and update care plans accordingly.
• Educate residents and families on care plans, medications, and health conditions.
• Ensure compliance with regulatory standards, policies, and best practices in senior care.
• Support transitions of care, including admissions, discharges, and transfers.
• Participate in quality improvement, safety initiatives, and staff huddles.
• Provide compassionate, person-centered support aligned with the organization's mission.
• Mentor and support care staff to promote high-quality resident care.
