Registered Nurse; RN Primary Care

Seeking an RN to provide direct patient care and support physicians in a primary care setting. Responsibilities include triage, assessments, care coordination, patient education, referrals, diagnostic review, and EHR documentation.

• Provide direct nursing care and patient assessments.
• Triage calls and walk-ins; provide clinical guidance.
• Review labs, imaging, EKGs, and diagnostic results.
• Coordinate care plans, referrals, and follow-ups.
• Educate patients on chronic disease management and wellness.
• Maintain accurate EHR documentation.
• Collaborate with physicians and multidisciplinary teams.
• Support quality improvement and patient safety.

• BSN required
• 2+ years nursing experience
  , preferably primary care/outpatient
• Current BLS required
• ACLS/PALS preferred
• Strong EHR, clinical assessment, communication, and care coordination skills
