Case Services and Outreach

Our client, a public health organization, is seeking a Case Services and Outreach professional to join their team. As a Case Services and Outreach specialist, you will be part of the Community Support Department supporting health outreach and enrollment initiatives. The ideal candidate will demonstrate strong communication skills, cultural competence, and a proactive attitude, which will align successfully in the organization.

* ** What's the Job?*
* + Conduct face-to-face outreach and enrollment services to beneficiaries in group and individual settings.

+ Present Managed Care Options to beneficiaries and assist them in navigating their healthcare plans.

+ Support beneficiaries through the enrollment and disenrollment processes.

+ Assist team members with daily operational duties and provide guidance to new staff.

+ Maintain positive relationships with county staff, community groups, and clients, including conducting ongoing training sessions.

** What's Needed?*
* + High School diploma, GED, or equivalent certification.

+ Knowledge of the community to be served and computer literacy, including MS Office applications.

+ Ability to learn new software quickly and type at least 20+ words per minute.

+ Valid CA driver's license and evidence of insurance, with the ability to travel throughout Santa Clara County and surrounding areas.

+ Excellent organizational, interpersonal, written, and verbal communication skills.
