On Lok PACE in San Jose seeks a Social Worker (MSW) to provide direct case management services for participants. The MSW collaborates with the Interdisciplinary team to assess, plan, coordinate, and refer social work services tailored to complex participant needs and family support.
With support from the Social Work Lead, the MSW conducts assessments, creates care plans, and delivers counseling while facilitating community resources.
