PATH operates services throughout California, connecting our clients to a comprehensive continuum of homelessness prevention, street outreach, employment preparation and placement assistance, individualized case management, supportive services, interim housing, and permanent supportive housing.

About the Job

As part of the Santa Clara team, the Case Manager role’s main function is to assist clients in breaking the cycle of homelessness by utilizing a “whatever it takes” approach. The position supports clients through all phases of homelessness—including on the street, interim housing, transitional housing, and permanent housing—and helps them access and maintain necessary services in healthcare, social services, interpersonal support systems, and a coordinated entry system (CES).

The role provides individualized supportive services by developing a plan that addresses strengths, barriers, health needs, income, and stable housing. The professional will identify areas of assistance to achieve the outlined goals and focus on housing and health as necessary outcomes for success.

Position

Responsibilities include:
  • Conduct intake and individualized needs assessment for all participants and collaborate to develop Individualized Service Plan (ISP) addressing housing goals, income, and participant‑identified goals (medical, mental health, substance use, financial resources, vocational, and social support needs).
  • Provide referrals, linkages, information, and support to resources that help participants achieve their ISP goals.
  • Complete program‑specific assessments (e.g., VI‑SPDAT).
  • Evaluate strengths and challenges to address short‑term and long‑term goals; conduct a 90‑day review with the participant after initial intake.
  • Maintain documentation standards as required by the Program and PATH policies, including timely case file notes and a complete, accurate, and current participant file with all required documents.
  • Maintain confidentiality of participant files per HIPAA and all applicable guidelines.
  • Develop therapeutic and/or professional relationships with clients and systems/partners to enhance efforts to engage and retain individuals with their housing goals.
  • Provide crisis intervention and risk assessment services focused on increasing safety and enhancing the client’s ability to solve problems independently, utilizing effective coping skills and self‑coordinate care, including services for individuals with chronic trauma. Use motivational interviewing and trauma‑informed care in service delivery.
  • Maintain current, thorough knowledge of community resources and utilize them to provide comprehensive, wrap‑around services to clients.
