Description
OVERVIEW:
The Intake Specialist Case Manager supports at-risk individuals and families by conducting screenings, coordinating referrals, and helping clients access community resources. The role includes assisting with rental applications, collaborating with housing staff, and advocating on behalf of clients with landlords to promote housing stability. Additionally, the position focuses on assessing client needs and delivering high-quality case management in partnership with the social services team.
DUTIES AND RESPONSIBILITIES:
- Screen clients for rental and/or utility assistance
- Determine the client eligibility
- Provide resources to families and individuals seeking services at the Family Services Department
- Screen at-risk individuals and families
- Provide internal referrals to connect clients with resources
- Establish and maintain relationships with other community agencies
- Attend all required staff, department, community meetings, and trainings
- Provide financial assistance for rental and deposit in coordination with the Housing Coordinator
- Provide case management to individuals and families
- Provide budgetary and financial counseling
