Vision Quest is seeking a Case Manager for Family Preservation and Prevention Ongoing Services. You will provide ongoing, family-centered case management to children, youth, and caregivers, coordinating referrals and maintaining compliant documentation.
You’ll engage families using trauma-informed approaches, develop service plans, conduct in-home visits, and collaborate with schools, courts, and providers to support safety and stability.
