Outreach & Enrollment Supervisor - San Leandro

Through the Program of All-Inclusive Care for the Elderly (PACE), we provide comprehensive medical care along with support for daily needs such as meals, transportation, and in-home assistance. We deliver coordinated clinical and social care in our centers and directly in participants' homes, creating a fully integrated experience that brings peace of mind and a true sense of belonging.

The Outreach & Enrollment Supervisor leads Habitat Health's Outreach and Enrollment team, driving participant growth through strategic leadership, operational excellence, and talent development. This role oversees the end-to-end outreach, intake, and enrollment functions, ensuring prospective participants receive a seamless, participant-centered experience from initial engagement through enrollment. Partnering closely with center leadership and cross-functional teams, the Outreach & Enrollment Supervisor develops high-performing teams, strengthens community partnerships, removes operational barriers, and leverages data-driven insights to achieve organizational growth goals while ensuring compliance with applicable regulatory requirements.

Core Responsibilities & Expectations for the Role:

  • Leads, coaches, and develops a high-performing Outreach and Enrollment team by fostering accountability, professional growth, continuous learning, and a culture of belonging.
  • Develops and executes outreach and enrollment strategies that drive participant growth, strengthen referral partnerships, and support organizational objectives.
  • Monitors team performance, operational metrics, and enrollment outcomes, using data and insights to identify opportunities, improve performance, and remove operational barriers.
  • Partners with center leadership and cross-functional teams to deliver a coordinated, participant-centered enrollment experience and support successful transitions into the PACE program.
  • Provides leadership and guidance on complex enrollment cases, participant eligibility, family dynamics, and operational challenges to support informed decision-making and positive participant outcomes.
  • Builds and maintains strategic relationships with healthcare providers, community organizations, referral partners, and other external stakeholders to expand referral networks and strengthen community engagement.
  • Leads community outreach initiatives, educational events, and local growth activities that increase awareness of Habitat Health's PACE program and support market growth.
  • Ensures compliance with applicable federal and state regulatory requirements, as well as Habitat Health policies and procedures, through operational oversight, quality monitoring, and continuous coaching.
  • Drives continuous improvement by identifying best practices, streamlining workflows, and implementing solutions that enhance participant experience, operational efficiency, and team performance.
  • Performs other related duties as assigned

Required Qualifications:

  • Bachelor's degree in Business Administration, Healthcare Administration, Public Health, Marketing, or a related field, or an equivalent combination of education and relevant leadership experience.
  • Minimum of three (3) years of experience in healthcare sales, outreach, enrollment, business development, care navigation, social services, community engagement, or a related healthcare environment.
  • Minimum of one (1) year of experience leading, coaching, mentoring, or developing individuals or teams, with demonstrated success in driving performance, developing talent, and achieving business objectives.
  • Experience working with older adults, frail elderly populations, caregivers, or within PACE, Medicare, Medicaid, managed care, or other value-based healthcare environments.
  • Demonstrated ability to develop and execute growth strategies while managing operational performance and achieving organizational goals.
  • Strong leadership, coaching, and talent development skills, with the ability to build engaged, high-performing teams and foster a culture of accountability, collaboration, and continuous improvement.
  • Strong communication, relationship-building, and collaboration skills, with the ability to effectively engage team members, participants, caregivers, providers, community partners, and organizational leaders.
  • Demonstrated ability to analyze operational and performance data to inform decision-making, prioritize initiatives, and improve organizational outcomes.
  • Experience using Microsoft Office, including Excel, and the ability to quickly learn Customer Relationship Management (CRM), Electronic Medical Record (EMR), and participant relationship management systems.
