We’re hiring a full-time Geriatric Physical Therapist to join our interdisciplinary care team in Livermore, CA
. You’ll play a vital role in our PACE (Program of All-Inclusive Care for the Elderly) — where your clinical expertise will directly improve mobility, safety, and independence for frail older adults.
- Evaluate the functional ability, mobility, and rehab needs of older adults
- Develop and implement individualized, evidence-based care plans
- Collaborate with physicians, nurses, social workers, and caregivers
- Educate patients, caregivers, and staff on safe mobility and equipment use
- Supervise PTAs and therapy aides within the care team
- Evaluates participants’ home environments and makes appropriate recommendations as needed.
- Degree from a CAPTE-accredited PT program
- 1+ years’ experience working with as a Licensed PT with the frail elderly population
- CPR & First Aid certification
- Valid driver’s license & reliable transportation for occasional local travel
- Reasonable caseload — Spend real time with each patient
