Job Opportunity – Registered Nurse Case Manager – California

Start Date: March 16, 2026
Location: San Leandro, California
Compensation: Details provided to qualified applicants
Schedule: Monday through Friday, 8:00 AM to 4:30 PM

Requirements:

• Active California Registered Nurse license
• Current BLS certification through the American Heart Association
• Minimum 2 years of recent RN Case Management experience
• Experience in LTAC (Long-Term Acute Care) or STACH (Short-Term Acute Care Hospital) settings required
• Demonstrated competency in MCG criteria and discharge planning across acute care environments
• Strong interdisciplinary team collaboration skills
