RoleScope
The Enrollment Specialist plays a key role in supporting Habitat Health’s growth by overseeing and managing the end‑to‑end enrollment journey for prospective participants. This role partners closely with the Outreach Specialist, Enrollment RN, and Interdisciplinary Team (IDT) to guide prospective participants and caregivers through the enrollment process, remove barriers, and support informed decision‑making. By coordinating enrollment activities and fostering trusted relationships, the Enrollment Specialist delivers a seamless, participant‑centered enrollment experience while ensuring compliance with organizational policies and regulatory requirements.
CoreResponsibilities and Expectations
- Build trusted relationships with prospective participants and caregivers to support enrollment growth and participant engagement.
- Drive the enrollment process as the case manager for prospective participants, including reviewing, processing, and updating applications and required documentation, distributing enrollment materials, and managing communications.
- Coordinate assigned enrollment cases by managing applications, documentation, participant communications, and required follow‑up activities in a timely and accurate manner.
- Partner closely with the Outreach team, Enrollment RN, IDT, and center leaders to deliver a coordinated, participant‑centered enrollment experience.
- Conduct enrollment visits in participants’ homes, the center, or virtually to assess needs, support eligibility activities, and advance the enrollment process.
- Identify and help resolve barriers to enrollment by proactively coordinating resources, maintaining communication, and facilitating a smooth transition into the PACE program.
- Maintain accurate enrollment documentation and participant records in accordance with organizational policies and regulatory requirements.
- Monitor enrollment activities and performance metrics, using data to prioritize work and contribute to continuous improvement.
- Contribute to community awareness of Habitat Health by representing the organization professionally and supporting outreach and referral development activities.
- Contribute to a program experience that participants want to join, a team culture that cares and creates joy, and an environment where all participants and team members belong.
- Perform other related duties as assigned.
- Bachelor’s degree or equivalent combination of education and/or relevant experience.
- Minimum of one (1) year of experience in healthcare sales, enrollment, customer success, business development, care navigation, or a similar relationship‑focused role, preferably within a healthcare environment.
- Demonstrated ability to build trusted relationships, educate prospective participants, and guide individuals through complex decision‑making with empathy and professionalism.
- Strong organizational skills with the ability to manage multiple priorities while maintaining attention to detail and ensuring timely follow‑through.
- Strong communication and collaboration skills, with the ability to effectively engage prospective participants, caregivers, referral partners, and cross‑functional teams to deliver a seamless enrollment experience.
- Demonstrated ability to work independently while collaborating across teams to achieve shared outcomes.
- Experience using Microsoft Office and the ability to quickly learn CRM, enrollment management, and participant relationship management systems.
- Proof of a valid driver’s license, reliable personal transportation, and automobile insurance that meets applicable state requirements, with the ability to travel throughout the service area.
