San Leandro Center (In‑person)

Hours / Shift

Full Time, Monday – Friday, 8:30AM – 5:30PM

Responsibilities

This position provides case management services to clients and their families referred through Stars’ Transition Age Youth (TAY) Program.

Responsibilities include:

Intensive case management, outreach, and engagement
Medication support and coordination
Providing mental health counseling and peer group facilitation
Connecting families with supportive services
Assisting TAY in transition to adult life in a trauma‑informed, culturally responsive manner

Qualifications

4 years of experience in a social service field
Associate degree plus 2 years of experience in a social service field
Bachelor’s degree in a social science field plus at least 6 months of experience in a social service field or a school internship in a social service field

Licenses and Certifications

Valid California Driver’s License and current automobile insurance, with a driving record meeting company driver eligibility policy.
