Requirements
- Current RN license in state of practice:

- Basic Life Support (BLS) certification:

- Associate Degree (A.S.) or equivalent:

- Three to five years nursing experience in an acute care hospital setting:

Roles & Responsibilities
- Assess appropriateness of admission status and level of care using Inter Qual criteria.
- Provide discharge planning services, coordinating with patients, families, and community agencies.
- Collaborate with medical staff to assure appropriate utilization of hospital resources.
- Document progress in the computer system and patient chart, ensuring timely responses to needs.
- Proactively assist in the development of patient care and teaching plans.
- Anticipate discharge needs and make appropriate referrals to resources within and outside the healthcare delivery system.
