Position: RN Case Management/Utilization Review
Facility: Adventist - Sierra Vista
Location: 1010 Murray Ave, San Luis Obispo, CA 93405
Contract Duration: 13 weeks (with possible extension)
Schedule: (4x10 ) Day shift (07:00 am–5:00 pm)
Weekly Hours: 40 hours
Position: RN II – Case Management / Utilization Review (Fully On-Site)
Role Overview:
The RN Case Manager is responsible for evaluating the appropriateness of admission status and level of care using Inter Qual criteria while ensuring effective utilization of hospital resources. The role involves coordinating discharge planning and collaborating with patients, families, community agencies, and the medical team to support high-quality patient care and smooth transitions from hospital to home or other care settings.
Schedule
- Shift: Day Shift (4 x 10-hour shifts)
- Hours: 7:30 AM – 6:00 PM
- Schedule: Rotating days (no fixed schedule)
- Weekend Requirement: Occasional weekend rotation
- On-Call: Not required
Key Responsibilities
- Assess admission status and level of care using Inter Qual criteria
- Coordinate discharge planning with patients, families, and community resources
- Collaborate with physicians and healthcare teams to ensure appropriate utilization of hospital services
- Facilitate timely patient care and safe discharge processes
- Maintain accurate documentation and communication with interdisciplinary teams
- Utilize hospital systems and software for case management activities
Minimum Requirements
- BLS Certification required
- Minimum 1 year of hospital-based Care Management experience
- Associate Degree in Nursing (ASN) or equivalent required
- Active RN License in the state of practice
- Basic computer proficiency (Word, Excel, Outlook, Internet navigation)
Preferred Qualifications
- Bachelor’s Degree in Nursing (BSN)
- 3–5 years of nursing experience in an acute care hospital setting
- Experience in Utilization Review, Discharge Planning, or Inpatient Case Management
Familiarity with Inter Qual criteria and Medicare Conditions of Participation
