Schedule: Day Shift – 4 x 10-hour shifts (07:30 AM – 06:00 PM)
Weekly Hours: 40
The RN II – Case Management / Utilization Review is responsible for assessing the appropriateness of patient admissions, level of care, and resource utilization in accordance with hospital policies and regulatory guidelines. This role supports discharge planning and care coordination to ensure patients receive appropriate, efficient, and high-quality healthcare services. The RN collaborates with physicians, interdisciplinary teams, patients, families, and community agencies to facilitate safe patient transitions and optimal clinical outcomes.
Key Responsibilities- Evaluate patient admission status and level of care using Inter Qual criteria.
- Conduct utilization reviews to ensure appropriate use of hospital resources.
- Develop and implement discharge plans in coordination with patients, families, and healthcare teams.
- Collaborate with physicians, nurses, and allied health professionals to coordinate patient care.
- Ensure compliance with Medicare Conditions of Participation and regulatory requirements.
Monitor patient progress and identify barriers to discharge.
- Shift: Day Shift
- Hours: 07:30 AM – 06:00 PM
- Pattern: 4 shifts per week (10 hours each)
- Weekend Rotation: Occasional
Education
- Associate Degree in Nursing (ASN) – Required
- Bachelor of Science in Nursing (BSN) – Preferred
- 3–5 years of nursing experience in an acute care hospital – Required
- 1+ year of hospital Care Management experience – Required
- Experience in utilization review, discharge planning, or inpatient case management – Preferred
- Current Registered Nurse (RN) license in the state of practice – Required
- Basic Life Support (BLS) certification – Required
