In this RN Case Management role, you’ll be at the core of patient success. Key responsibilities include thorough clinical assessments and risk stratification, developing individualized care plans, coordinating services across multidisciplinary teams, arranging safe and timely discharges, and ensuring smooth transitions across inpatient, outpatient, and community settings. You’ll perform utilization review and payer communications with professionalism and accuracy, monitor outcomes through data-driven metrics, and educate patients and families about treatment options, medications, and self-management strategies.

Your leadership will shine in coordinating with social workers, physicians, therapists, and home health providers to optimize resources, reduce readmissions, and enhance patient satisfaction. The weekly pay range is $2,596 to $2,787.
