Aevum Hospice is seeking a compassionate and experienced
Registered Nurse (RN) Case Managerto join our hospice care team. The ideal candidate has hospice experience, strong case-management skills, and a genuine commitment to supporting patients and families during end-of-life care.
- Manage and coordinate care for an assigned hospice patient caseload
- Complete comprehensive nursing assessments and develop individualized plans of care
- Monitor and document changes in condition, symptom burden, functional status, and disease progression
- Provide pain and symptom-management interventions in collaboration with the hospice physician and interdisciplinary team
- Educate and support patients, families, caregivers, and facility staff
- Coordinate medications, medical equipment, supplies, and other hospice services
- Communicate effectively with physicians, facilities, pharmacies, and members of the interdisciplinary team
- Participate in interdisciplinary group meetings and assist with care planning
- Complete accurate, timely, and Medicare-compliant clinical documentation
- Participate in the on-call rotation as assigned
- Current and unrestricted California Registered Nurse license
- Previous hospice nursing experience strongly preferred
- Case-management experience required
- Knowledge of hospice eligibility, plans of care, recertification, and Medicare documentation requirements
- Strong clinical assessment, communication, organization, and time-management skills
- Ability to work independently while remaining engaged with the interdisciplinary team
- Valid driver’s license, reliable transportation, and current automobile insurance
- Current CPR certification
- Compassionate, dependable, and comfortable caring for patients with advanced and terminal illnesses
