Position: Registered Nurse - Hospice Case Manager (Paso Robles and SLO Areas)

Aevum Hospice is seeking a compassionate and experienced

Registered Nurse (RN) Case Managerto join our hospice care team. The ideal candidate has hospice experience, strong case-management skills, and a genuine commitment to supporting patients and families during end-of-life care.

Key Responsibilities
  • Manage and coordinate care for an assigned hospice patient caseload
  • Complete comprehensive nursing assessments and develop individualized plans of care
  • Monitor and document changes in condition, symptom burden, functional status, and disease progression
  • Provide pain and symptom-management interventions in collaboration with the hospice physician and interdisciplinary team
  • Educate and support patients, families, caregivers, and facility staff
  • Coordinate medications, medical equipment, supplies, and other hospice services
  • Communicate effectively with physicians, facilities, pharmacies, and members of the interdisciplinary team
  • Participate in interdisciplinary group meetings and assist with care planning
  • Complete accurate, timely, and Medicare-compliant clinical documentation
  • Participate in the on-call rotation as assigned
Qualifications
  • Current and unrestricted California Registered Nurse license
  • Previous hospice nursing experience strongly preferred
  • Case-management experience required
  • Knowledge of hospice eligibility, plans of care, recertification, and Medicare documentation requirements
  • Strong clinical assessment, communication, organization, and time-management skills
  • Ability to work independently while remaining engaged with the interdisciplinary team
  • Valid driver’s license, reliable transportation, and current automobile insurance
  • Current CPR certification
  • Compassionate, dependable, and comfortable caring for patients with advanced and terminal illnesses
