Family Care Network, Inc. (FCNI) is a mission-driven nonprofit serving children, youth, adults, and families across San Luis Obispo and Northern Santa Barbara Counties. Our team works collaboratively with local county and community-based agencies to provide compassionate, trauma-informed services; including evidence-based mental health treatment, case management, housing and homelessness support, and family-based interventions across Child Welfare, Behavioral Health, and Juvenile Justice systems of care—as well as support for individuals and families experiencing complex challenges who fall outside of these social services.

We are currently looking to add a new member to our dynamic team for the position of a Case Manager in our ECM program. The Case Manager serves as the primary point of contact for clients, coordinating care across multiple systems—including medical, behavioral health, and social service providers. This role involves conducting needs assessments, developing individualized care plans, advocating for client needs, facilitating access to resources such as housing and benefits, and ensuring that care is person-centered.
The Case Manager works closely with the client’s care team to promote positive health outcomes.

SCHEDULE:
- FCNI office hours are Monday through Friday, 8:30 a.m. to 5:00 p.m. Staff must maintain flexibility in their schedules to meet client needs and accommodate required meetings, supervision, and other program-related responsibilities. (Shifts may include the following: weekends, holidays, splits and evenings).
- Pay Range: $23.60 -$35.40 per hour
- Full Time, Non-Exempt classification per FLSA
- Spanish Bilingual differential - 10% increase on the employee's hourly base rate of pay (if applicable)
- Mileage reimbursement $0.76cents per mile
- Eligible to enroll into benefits on the 1st of the month following 60 days of employment. These are all high quality and comprehensive plans.
- FCNI will cover 75% of employees’ premium and 60% of employee's dependent’s premium on the primary plan offered-Cigna PPO Health Benefit, Vision and Dental PPO.

- High School diploma or equivalent required. Associate or Bachelor’s degree preferred in Social Work, Psychology, or related field.
- One (1) year of direct experience in a Behavioral Health or Social Services setting such as outpatient clinics, facilitates and/or related community-based organizations, preferred
- Must be computer literate with proficiency in Microsoft Office Suite and knowledge of database and reporting tools
- Excellent communication skills (verbal, written, and interpersonal)
- Excellent time management, organizational and multi-tasking skills
- High level of accuracy and attention to detail
- Ability to maintain discretion and confidentiality with sensitive information
- This position requires frequent driving as an essential function of the job.
