Position Summary
The Gary and Mary West PACE (GMWP) Clinic RN is responsible for coordinating and providing direct and indirect nursing care to participants at the center as well as in participants’ homes. The RN works closely with the PACE Physician, clinic staff, and the interdisciplinary team (IDT) to manage ongoing care and complex clinical situations and to support the operation of the GMWP primary care clinic.

- Conduct clinic nursing assessments to determine participants’ care needs, preferences and goals and actively participate in IDT meetings to develop care plans.
- Visit participants’ homes to deliver nursing interventions.
- Deliver and document nursing interventions as agreed upon in care plans, including maintaining a safe environment, responding promptly to physician orders, and administering medications and ordered tests correctly.
- Remain alert to input from team members, participants and caregivers, and update the IDT promptly of any changes in condition or status.
- Work with the PACE Physician and other IDT members to manage smooth care transitions between settings such as hospitals, skilled nursing facilities and home.
- Instruct participants, families and caregivers about disease processes, self‑care techniques and prevention strategies.
- In coordination with the Marketing Team, support enrollment of prospective participants into the program.
- Participate in end‑of‑life care coordination and support.
- Provide backup coverage to other GMWP nursing functions.
- Complete all required documentation in a timely and accurate manner.
- Protect privacy and maintain confidentiality of all company procedures and information about employees, participants and families.
- Follow GMWP policies and procedures and participate in required quality improvement activities, staff training and meetings.
- Perform other duties as assigned.
- Graduate of an accredited school of nursing; BSN preferred.
- Unencumbered California Registered Nurse (RN) license.
- Current Basic Life Support (BLS)/First Aid certification required.
- Original valid driver’s license with proof of auto insurance; personal vehicle required.
- Minimum of two years of clinical nursing experience with frail or elderly populations.
- Knowledge and skills to treat frail, elderly participants and manage complex clinical situations.
- Highly motivated, self‑directed, able to execute tasks in a fast‑changing environment and make sound decisions in emergencies.
- Excellent clinical, organizational and communication skills working with seniors, their families and interdisciplinary team members.
- Experience and competency working with people from diverse backgrounds.
- Commitment to unlocking the full potential of vulnerable seniors.
- Bilingual Spanish‑English preferred.
Employees must have medical clearance for communicable diseases and up‑to‑date immunizations before having direct participant contact.
