Gary and Mary West PACE is seeking a Nurse Practitioner to deliver primary care and continuous medical coverage for PACE participants. You will work within an interdisciplinary team, in collaboration with PACE physicians and community-based providers, to assess and manage chronic conditions in a geriatric population.
You will perform comprehensive in-home and center-based evaluations, coordinate care plans, participate in 24/7 on-call coverage, and ensure compliance with PACE standards.
