You will collaborate with the PACE Physician, RN, clinic staff and IDT to manage ongoing care and support clinic operations.
Requirements include CA LVN license, BLS/First Aid CPR, driver license, and a clean driving record. Two years of nursing experience with frail or elderly populations; bilingual Spanish-English
