Gary and Mary West PACE (GMWP) Clinic RN coordinates and provides direct and indirect nursing care to participants at the center and in their homes. Works closely with the PACE physician, clinic staff, and interdisciplinary teams to manage ongoing care and support clinic operations.
Responsibilities include conducting nursing assessments, home visits, medication administration, and documentation. Bilingual Spanish-English preferred; BSN preferred; CA RN license required.
