Philanthropists Gary and Mary West founded West Health with the mission of enabling seniors to age in place with high‑quality, affordable health and support services. The Gary and Mary West PACE center delivers medical, social, and behavioral services to frail elderly in North County San Diego.
Position SummaryThe GMWP Clinic RN coordinates and provides direct and indirect nursing care to participants at the center and in participants' homes. The RN works closely with the PACE Physician, clinic staff, and interdisciplinary teams to manage ongoing care, complex clinical situations, and support the operation of the primary care clinic.
Essential Functions- Conduct clinic nursing assessments to determine care needs, preferences, and goals; actively participate in IDT meetings to develop care plans.
- Visit participants' homes to deliver nursing interventions.
- Deliver and document interventions per care plans, including medication administration, tests, and treatments.
- Remain alert to input from team members, participants, and caregivers, promptly updating the IDT of any changes.
- Work with the PACE Physician and IDT to manage smooth transitions between settings.
- Educate participants, families, and caregivers on disease processes, self‑care, and prevention.
- Support enrollment of prospective participants in coordination with Marketing.
- Participate in end‑of‑life care coordination and support.
- Provide backup coverage to other nursing functions.
- Complete all required documentation timely and accurately.
- Protect privacy and maintain confidentiality of all company procedures and sensitive information.
- Follow GMWP policies, participate in quality improvement activities, staff training, and meetings.
- Perform other duties as assigned.
- Graduate of an accredited nursing school; BSN preferred.
- Unencumbered California Registered Nurse (RN) license.
- Current Basic Life Support (BLS) / First Aid certification.
- Valid driver’s license with proof of auto insurance.
- Minimum two years of nursing experience with a frail or elderly population.
- Clinical knowledge and skills to treat frail, elderly participants and manage complex situations.
- Highly motivated, self‑directed, able to execute tasks in a rapidly changing environment and make sound decisions in emergencies.
- Excellent clinical, organizational, and communication skills with seniors, families, and interdisciplinary team.
- Experience working with people from diverse backgrounds.
- Commitment to unlocking the full potential of vulnerable seniors.
- Bilingual Spanish–English preferred.
Employees must have medical clearance for communicable diseases and up‑to‑date immunizations before direct participant contact.
