The incumbent is responsible for ensuring the delivery of efficient and effective nursing care while achieving positive clinical outcomes and resident/family satisfaction in accordance with accepted standards of practice, state and federal regulations, and licensing requirements. The role operates within the scope of practice defined by the state Nurse Practice Act and oversees resident care and direction of nursing care during the assigned shift.
Tasks include staff assignments, mentoring and educating nursing personnel, and working with physicians and other medical professionals.
- Conduct daily nursing functions in accordance with company, state, federal, and local rules, regulations, and guidelines.
- Admit, transfer, and discharge residents with documentation regarding care and service provided.
- Participate in the orientation of new residents/family members to the facility.
- Participate in facility surveys (inspections) as scheduled.
- Ensure delivery of high‑quality, positive clinical outcomes and resident/family satisfaction during the shift.
- Serve as a resource to all staff within the facility regarding the quality of services provided.
- Administer medications and treatment timely and in accordance with facility policy; receive, transcribe, and carry out physician orders when permitted by the Nurse Practice Act.
- Effectively manage nursing personnel on shift to limit overtime and agency use.
- Assess the performance of nursing staff relative to unit standards of care and goals.
- Conduct unit huddles to promote communication and provide updates to the Director of Nursing.
- Participate in shift‑to‑shift communication between incoming and outgoing nursing staff.
- Coordinate shift assignments, resident assignments, and rest and meal breaks for Certified Nurse’s Aides.
- Collaborate with other departments to provide timely, effective care consistent with individuals’ needs, choices, and preferences.
- Assume nursing administrative responsibilities for weekend and holiday coverage as requested.
- Stay and work beyond scheduled shift if needed to meet state staffing requirements or residents’ needs.
- Ensure timely and accurate compliance with the MDS process and reflect the resident’s plan of care.
- Ensure Point Click Care (PCC) is utilized according to company processes.
- Ensure physician orders are followed as prescribed.
- Manage medication supply and handling in accordance with company policy.
- Notify the resident’s attending physician and family or responsible party promptly of any significant change in the resident’s health condition.
- Report incidents, accidents, and adverse events to the Administrator and Director of Nursing; ensure complete documentation, investigation, reporting, and resolution in accordance with company policies, federal, and state regulations.
- Investigate resident/family complaints and take appropriate actions to bring them to resolution.
- Demonstrate knowledge and appropriate use of the Company Infection Control Manual.
- Evaluate the effectiveness of care interventions, identify problems and trends, and develop alternative interventions.
- Perform other duties, responsibilities, and activities that may change or be assigned at any time.
- Graduate of an approved Licensed Vocational program and licensed in the state of practice.
- Minimum of 2 years of nursing experience in a Skilled Nursing Facility is preferred.
- Current BLS/CPR certification (required for all SNFs; as required by state for ALFs).
