RN Patient Care Manager - Hospice

We are seeking an RN Patient Care Manager for our Hospice team at CHRISTUS Hospice – Central Texas in San Marcos, TX. The role supports patient care activities, ensuring safe and efficient delivery in accordance with regulatory standards.

• Assist Executive Administrator/Executive Director/Director of Clinical Operations with all patient care activities to ensure delivery of safe and efficient patient care that adheres to relevant standards.
• Ensure compliance with hospice policies and procedures as well as local, state, and federal regulatory requirements governing the practice of the assigned team.
• Lead, inspire, and motivate staff to provide exceptional care.
• Ensure all staff are oriented and supported to be successful in their job performance.
• Act as liaison between staff, patients, and the medical community.
• Promote and direct assigned team regarding quality of care and safety of patients and staff.
• Engage in thorough problem resolution and complaint investigation.
• Provide in-services to staff to promote patient health and safety, familiarize staff with regulatory issues and agency policy, and address identified educational needs.

• At least four years of full‑time RN experience or equivalent.
• Current RN licensure in the state of practice.
• Minimum one year of clinical experience.
• Current CPR certification.
• Current driver’s license, valid vehicle insurance, and access to a dependable vehicle or public transportation.
