Helping Central Texas improve economic self-reliance through a wide range of services and community partnerships.

Provides outreach and recruitment support for the Home Visiting Program. Supports families through comprehensive case management services that will support caregivers to foster their child's development and assist caregivers to attain their personal goals. Conducts home visits using Parents as Teachers curriculum. Reports to Home Visiting Program Director.

DUTIES/RESPONSIBILITIES

May include the following:

Outreach & Recruitment
  • Implement procedures for the recruitment of families in Hays and Caldwell Counties.
  • Work directly with program leadership and recruitment subcommittee to implement agreed upon strategies.
  • Coordinate direct recruitment events and opportunities.
  • Communicate directly with families who have registered for the Home Visiting Program.
  • As needed, conduct program intake with families and enter into the data management system.
  • Share all relevant information regarding program expectations with families.
  • Share information about family dynamics with leadership and/or assigned Parent Educator.
  • Assist program leadership to ensure compliance with policies, procedures, and regulations established by regulators, funding agencies, and Community Action, Inc. as they apply to Home Visiting Program. Specifically, monitor progress on MIECHV Benchmarks, DFPS Outputs & Outcomes, and PAT Essential Requirements.
  • Assist in the development of procedures for the Home Visiting Program.
  • Participates in workshops, in-service training and makes use of all personal and professional growth opportunities.
  • Develop and maintain positive, open and productive team partnerships with families, community partners and staff while maintaining professional boundaries.
Parent Education
  • Conduct home visits and group connections curriculum with young children and their families using the Parents as Teachers.
  • Conduct screening and ongoing assessment of young children’s development and family well-being
  • Conduct caregiver screenings and assessments to support parenting and family well-being.
  • Obtain and maintain Parent’s as Teachers (PAT) certification at agency’s expense.
  • Make referrals to needed resources and assist families in accessing resources as necessary and appropriate.
  • Meet monthly with the Home Visiting Program Director to assign priorities to families on your caseload (more frequently as needed).
  • Maintain accurate and timely documentation by funder and PAT model standards.
  • Flexibility with work schedule and location as needed based on program needs.
  • Adhere to the Social Work Code of Ethics.
  • Report all suspected child abuse to the Child Abuse Hotline at and to Home Visiting Program Director.
  • Other duties as assigned.
  • Submits time sheets accurately and on time to supervisor.
  • Submits mileage accurately and on time to supervisor.
  • Attends all staff and other meetings as assigned.

  • Maintains confidentiality: records, client services, and staff.
  • Adheres to agency's best practices related to time and leave.
  • Dress is appropriate to work environment.

REQUIREMENT/QUALIFICATIONS Preferred:
  • Bachelor’s Degree in Social Work
  • 2 years experience working with families in their homes.
