Role
Description:
This is a full-time home health role for a Physical Therapist at Care Mo Home Health. The Physical Therapist will be responsible for creating customized treatment plans, conducting thorough patient evaluations, maintaining accurate patient records, and collaborating with interdisciplinary care teams. The Physical Therapist will also be responsible for providing in-home care and rehabilitation services to patients.
