We are seeking an Admissions RN to provide information about services to potential agency patients and admit patients to the agency’s total care category of service in coordination with Clinical Team Manager(s).

Schedule:

Friday, Saturday, Sunday & Monday; 12:00 pm – 8:30 pm

Location:

San Ramon and San Mateo County, with additional coverage to Alameda & Contra Costa Counties as needed.

Mission Hospice & Home Care follows all CDPH vaccine requirements for healthcare personnel.

Essential Duties & Responsibilities include (but are not limited to):

• Screen and process inquiries for admission to MHHC’s total care category.
• Determine patients’ eligibility for hospice care.
• Make telephone contact and/or assessment visits with potential hospice patients to provide information on agency services and/or assess patient status.
• Assist with implementation and coordination of services required for discharge of patients from hospitals into the agency’s program of care.

Education / Experience

• One year experience as a professional nurse in the past three years or a Bachelor’s degree in nursing from an accredited School of Nursing.
• Experience in acute care / home health and/or hospice setting preferred.

Certificates, Licenses, Registrations

• Current license as a registered professional nurse in the State of California.
• Public Health Nursing certification highly desirable.
• HPNA certification preferred.

Pay Range
: $55.00 – $69.50 hourly
