-Limited Term to support the Trauma-Informed Approach Public Health Nurse Home Visiting Program. The Trauma-Informed Approach in Public Health Nursing (TIA PHN) program is a home-visiting public health nursing model that supports families with children (0-18) who may be experiencing trauma, stress, or social challenges.
The PHN home visitors are responsible for providing comprehensive nursing services to children and their families using an evidenced-informed home visiting program. The PHNs will work to promote the health of families and their young children, strengthen parent-child relationships and support a healthy environment for the entire family. The PHNs will make referrals to community and other agencies, coordinate care with hospitals and medical providers, educate and counsel parents.
The PHN home visitors are responsible for maintaining the highest standards in clinical nursing practice and adherence to policies, procedures, program visit guidelines, and standards.
While the primary assignment will be in the TIA PHN Program, successful hires may support other Family Health Services programs and activities based on departmental needs.
Responsibilities include:
* Acts as primary case manager and performs monthly in-person home visits with enrolled families at minimum.
* Counsel, teach, and coordinate health and social services provided to client/family.
* Completes initial and periodic health and social assessments (at enrollment and at minimum every 6 months); thereafter, screens adults' caregivers for mood disorders and intimate partner violence (IPV) and ensures that all enrolled children complete a developmental screen.
* Initial assessment, periodic review(s), and exit/closure visits for all enrolled family members.
* Provides information about ACEs, initiates ACEs conversation, and delivers the TIA PHN curriculum to enrolled primary caregivers.
* Address, track and record model outcome measures through enrollment and at closure.
* Collaborate with other team members to provide services, referrals and health education as appropriate based on their scope of practice, including providing nursing consultation for children and families as assigned.
* Promotion of positive family health practices through nursing education.
* Utilize trauma-informed care when supporting and visiting families and children.
* Support of Parent/Child dyad through the use of relationship-based child development curriculum and activities.
* Incorporate a strengths-based approach to develop therapeutic relationships with families.
* Apply motivational interviewing concepts to support behavioral change.
* Timely documentation in multiple EHR systems for nursing practice and data collection for evidence-based programming.
* Attend & participate in-person mandatory team meetings and case conferences as well as Reflective Supervision, 1:1 meetings with the Public Health Nurse Supervisor., and HV All Staff Meetings.
* Adhere and follow the Family Health Expectations and the County Employee Expectations.
* Adhere to the nursing process as indicated by the CA Board of Registered Nursing.
Currently, there are two limited-term, full-time vacancies.
The ideal candidates would possess the following:
* Proven ability to establish interpersonal relationships with individuals and families from diverse socioeconomic and cultural backgrounds.
* Strong background in normal child growth and development and parent-child relationships with children from birth to 18 years old.
* The ability to engage families from a family-centered, strengths-based model.
* Demonstrates the ability to learn and practice supportive parenting and relationship promotion between parent and child.
* Knowledge of trauma-informed care and adverse childhood experiences.
* The ability to perform health and developmental assessments for adults and children.
* Flexible in case assignments in various areas of San Mateo County.
* Excellent case management skills.
* Excellent written and verbal communication skills.
* Basic computer skills, electronic health record experience preferred.
* High degree of autonomy and flexibility. Ability to impose structure throughout the workday, both in between and during visits while also being flexible with changing work requirements or client needs.
